FACTORS ASSOCIATED WITH A MORE AGGRESSIVE BIOLOGICAL BEHAVIOR IN PAPILLARY MICROSCOPIC THYROID CARCINOMA (PMTC) Papillary microscopic thyroid carcinoma (PMTC) is defined as papillary thyroid cancer (PTC) measuring < 10 mm. In the past, PTMC was more commonly an incidental finding on histopathological examination after thyroidectomy performed for a presumably benign disease (such as multinodular goiter). Currently, however, PMTC is increasingly diagnosed preoperatively, based on the findings of high-resolution ultrasography and fine-needle aspiration (FNA) cytology. PMTC are believed to have an excellent prognosis, but some may behave more aggressively than previously appreciated. Indeed, lymph node and distant metastases may be present in up to 30 % and 0.6 % of patients at the time of diagnosis. Moreover, 3.5 % of patients will experience disease recurrence. Το θηλώδες καρκίνωμα θυρεοειδούς ( PTC ) (η συνηθέστερη μορφή καρκίνου θυρεοειδούς) έχει εξαιρετική πρόγνωση και αρκετά ‘καλοήθη’ βιολογική συμπεριφορά, καθώς η συνολική 5ετής επιβίωση υπερβαίνει το 97 %. T ο μέγεθος του PTC έχει προγνωστική σημασία, με τους μικρότερους σε μέγεθος όζους να έχουν καλύτερη πρόγνωση. Σαν θηλώδη ‘μικροκαρκινώματα’ (ή μικροσκοπικά θηλώδη καρκινώματα, Papillary Microscopic Thyroid Cancer , PMTC ) ορίζονται τα PTC διαμέτρου < 10 mm . Στο παρελθόν, τα P ΜΤ C βρίσκονταν συχνά κατά την ιστολογική εξέταση των θυρεοειδών που είχαν αφαιρεθεί για υποτιθέμενη καλοήθη πάθηση (συνηθέστατα πολυοζώδη βρογχοκήλη). ΡΑΔΙΕΝΕΡΓΕΙΑ ΚΑΙ ΚΑΡΚΙΝΟΣ ΘΥΡΕΟΕΙΔΟΥΣ – ΜΑΘΗΜΑΤΑ ΑΠΟ ΤΟ CHERNOBYL (ΟΥΚΡΑΝΙΑ, 1986) ΚΑΙ ΤΗΝ FUKOSHIMA (ΙΑΠΩΝΙΑ, 2011) CHERNOBYL (UKRAINE). Την 26 Απριλίου 1986, μετά από ατύχημα στο εργοστάσιο παραγωγής πυρηνικής ενέργειας στο Chernobyl , αποδεσμεύθηκαν στην ατμόσφαιρα τεράστιες ποσότητες ραδιενεργών υλικών (και ραδιενεργού ιωδίου). Το ατύχημα αποδόθηκε σε σειρά κατασκευαστικών λαθών (όσον αφορά τη λήψη προφυλακτικών μέτρων [απουσία ειδικών φίλτρων/προστατευτικών φραγμών]), αλλά και σε λάθη των εργαζομένων στο εργοστάσιο. Η περιβαλλοντική καταστροφή ήταν τεράστια, καθώς λόγω θέσης αλλά και λόγω των ανέμων, το ραδιενεργό νέφος κάλυψε μεγάλες γειτονικές εκτάσεις σε χώρες όπως η Ουκρανία, η Λευκορωσία, η Ρωσία αλλά και σε όλη σχεδόν την Ευρωπαϊκή ήπειρο (μέχρι και την Ισπανία). RADIATION/NUCLEAR EXPOSURE AND THYROID CANCER – LESSONS LEARNED FROM TWO NUCLEAR PLANT ACCIDENTS (Chernobyl [Ukraine] 1986 and Fujushima [Japan] 2011) In 1986, after the accident at the nuclear power plant at Chernobyl (Ukraine) huge amounts of radioactivity was released in the atmosphere, including large amounts of radioactive iodines. The radiation dose to the thyroid gland was high in Belarus, Ukraine, and South Russia since the uptake of radioiodines in the thyroid gland was high due to iodine deficiency and no iodine prophylaxis. Because of wind during the days after the accident, the radiation cloud spread over large territories in extended areas of Europe. Local authorities failed to organize a timely response to the disaster. DRAMATIC INCREASE OF THE INCIDENCE OF THYROID CANCER- HOW CAN BE EXPLAINED? Thyroid cancer (TC) is the most rapidly increasing malignancy in the Western World. In a report based upon the SEER database, the incidence of papillary thyroid cancer (PTC) increased from 4.8 (2002) to 14.9 (2012) per 100.000 (x 3). This increasing incidence is attributed to the: I.Improved identification of small TC due to more frequent and widespread use of high-resolution ultrasonography, combined with fine-needle aspiration (FNA) cytology. II.Increased reporting of incidental microcarcinomas (TC < 10 mm) in thyroids removed for presumably benign disease (for example, nodular goiter). III.Increased iatrogenic (“diagnostic” or therapeutic) radiation exposure (for example in patients with other malignant diseases, such as Hodgkin disease). ΠΟΥ ΟΦΕΙΛΕΤΑΙ Η ΣΥΝΕΧΗΣ ΑΥΞΗΣΗ ΤΗΣ ΣΥΧΝΟΤΗΤΑΣ ΤΟΥ ΚΑΡΚΙΝΟΥ ΘΥΡΕΟΕΙΔΟΥΣ; Η συχνότητα του καρκίνου θυρεοειδούς αυξάνεται με ταχύτερο ρυθμό από οποιαδήποτε άλλη κακοήθη πάθηση στις δυτικές χώρες. Σε μία πρόσφατη αναφορά στηριγμένη στη βάση δεδομένων SEER , η συχνότητα (επίπτωση) του θηλώδους καρκίνου θυρεοειδούς πρακτικά τριπλασιάστηκε μεταξύ 1975 και 2012 (από 4.8 σε 14.9 ανά 100.000 πληθυσμού). HURTHLE CELL CARCINOMA OF THE THYROID – WHY IS IT REPORTED SEPARATELY? Oncocytic follicular cells of the thyroid (the so-called H ü rthle cells) are large polygonal cells with marked eosinophilic, granular cytoplasm reflective of overly abundant mitochondria and can be observed in both benign and malignant conditions of the thyroid gland. Focal oncocytic trans-differentiation results in neoplasia consisting predominantly of H ü rthle cells that can represent benign H ü rthle cell adenomas or malignant H ü rthle cell carcinoma (HCC). H ü rthle cells are also seen in the oncocytic variant of the papillary thyroid carcinoma.
