NON-INVASIVE
FOLLICULAR THYROID NEOPLASM WITH PAPILLARY-LIKE NUCLEAR FEATURES (NIFTP) – no
so frequent and not always with a “very benign” biological behavior
Once described
as “encapsulated follicular variant of papillary thyroid cancer [EFVPTC]”, this
clinical entity is currently renamed as ‘noninvasive follicular thyroid
neoplasm with papillary-like nuclear features [NIFTP]’ to indicate its benign
biological behavior. In a recent report from Toronto, Canada, the incidence was
2.1 % of all papillary thyroid cancers (PTCs), a incidence much lower than that
is commonly reported in North American and European populations (16 – 23 %).
This lower incidence has been reported in recent publications from Asia. Currently
available data indicate that ΝIFTP has in general a benign biological behavior and an excellent
prognosis; however, adverse oncologic events (such as distant or neck lymph
node metastases) have been reported in about 6 % of these patients, an occurrence
not consistent with a benign diagnosis. Based on such data, the American
Thyroid Association has recently published a statement suggesting that a NIFTP
diagnosis should not affect the management and follow-up of patients with very
low-risk well-differentiated thyroid cancer.
