HYPERTHYROIDISM - FACTORS FAVORING SURGERY OVER
RADIOIODINE ABLATION THERAPY (RIA)
Antithyroid drugs (ATDs) or surgery (near-total
thyroidectomy) is preferred over RAI in Europe and Japan, while RAI has been
the preferred therapy in the US. Surgery should be preferred over RIA in
-women planning a pregnancy in < 6 months -pregnancy
or breastfeeding
-large toxic goiters (> 80 gr)
-pressure symptoms
-low uptake of RAI
-suspicion or presence of thyroid malignancy
-large or multiple thyroid nodules
-nonfunctioning or hypo functioning thyroid
nodules
-coexisting hyperparathyroidism
-younger patients (especially children)
-requirement for immediate control of the
disease
-active ophthalmopathy
-patients preferring surgery as a method for
definitive treatment
