HYPERPARATHYROIDISM;
IS ROUTINE INTRAOPERATIVE SERUM PARATHORMONE LEVELS MEASUREMENT NECESSARY?
Intraoperative
serum parathormone levels measurement (IOPTH) is recommended for patients
undergoing selective (targeted or minimally invasive) parathyroidectomy on the
basis of a single preoperative localization study or when preoperative
localization with sestamibi scan and ultrasound is not concordant. IOPTH is
also recommended for patients undergoing reoperative parathyroidectomy for persistent
/ recurrent hyperparathyroidism either to lateralize hyperfunctioning parathyroid
tissue (internal jugular vein/s sampling) when preoperative localization is
uncertain or to verify success of surgery thereby reducing the need for
continued exploration in the scarred neck.
