ECTOPIC
INTRATHORACIC THYROID
PATIENT:
40-yr old (female) with multinodular goiter and ectopic intrathoracic thyroid
PREOPERATIVE
INVESTIGATION: Included sonography, scintigraphy (I-131), chest radiography,
chest computed tomography and revealed multinodular goiter at the orthotopic
(cervical) thyroid and the presence of intrathoracic ectopic thyroid tissue, at
the right side of the posterior mediastinum, behind the great vessels,
extending to the level of trachea bifurcation.
PROCEDURE:
Orthotopic thyroid was removed through the cervical incision. However, removal
of the intrathoracic thyroid was not possible through the cervical incision and
a right lateral thoracotomy was performed. At surgery, the intrathoracic
thyroid was not connected with the cervical thyroid and its blood supply was
from intrathoracic vessels, thereby establishing the diagnosis of ectopic
intrathoracic thyroid. The ectopic intrathoracic thyroid was removed through
the right lateral thoracotomy.
POSTOPERATIVE
COURSE: Uneventful, patient discharged on postoperative day 4
HISTOLOGY:
Colloid multinodular goiter in both the cervical and the ectopic intrathoracic
thyroid.
