THYROID
TISSUE WITHIN A LYMPH NODE OF THE NECK; CLINICAL IMPORTANCE
True embryologic
rests of normal thyroid tissue (result of migration error) have been described
in many ectopic locations, including lymph nodes in the lateral neck. In the
presence of a substantial amount of thyroid tissue within a lymph node in the lateral
neck should raise the suspicion of a nodal metastasis of a well-differentiated
thyroid cancer, which is often undiagnosed (occult thyroid cancer). In contrast,
nodal embryologic rests of thyroid parenchyma are small, contain only a few
thyroid follicles, without nuclear features suggestive of papillary carcinoma,
and are located in the capsular region of the node.
