SYNDROME (HBS) FOLLOWING SUCCESSFUL PARATHYROIDECTOMY FOR PRIMARY
HYPERPARATHYROIDISM (PHPT)
HBS
following successful parathyroidectomy for PHPT is not uncommon occurring in
about 10 – 15 % of patients. HBS is characterized by prolonged severe
postoperative hypocalcemia and hypophosphatemia.
HBS is the result of accelerated remineralization of bone following sudden
decreased of parathormone after successful parathyroidectomy. HBS requires
aggressive management, often with intravenous calcium supplementation. HBS typically
is associated with overt clinical manifestations of hypocalcemia, which may
occasionally be dramatic (such as muscular spasms, including the ‘hand of the
obstetrician’). The patient should be well informed about the possibility of HBS
complicating postoperative course in order to recognize early clinical
manifestations of this syndrome, enabling early and effective management.
